Hypertrophic right L5 transverse process with pseudo articulation with the sacrum.
The facet joints are unremarkable.
Lumbosacral transitional vertebra (LSTV) is a commonly encountered finding. Its recognition is straightforward, but can be overlooked. Meanwhile, its clinical significance is in doubt.
The presence of low back pain along with a LSTV makes this syndrome, but it remains controversial.
According to Castellvi classification of lumbosacral vertebrae, this case is consistent with type IIa LSTV.
