Crossed fused renal ectopia with hydronephrosis
Subacute onset of right upper quadrant pain. Surgeon exam didn't reveal pathological symptoms, only RUC soreness.

There is a left-to-right crossed fused renal ectopia with a hydronephrotic transformation of the upper part. There is no obviously differentiated upper ureter, the inferior ureter is seen on coronal images.
Only single renal artery supplying fused kidneys but at the same time, two renal veins are seen. Other organs and retroperitoneal tissues look unremarkable.
It's a rare urinary tract malformation with incidence is around 1 out of 1000 births. And the more rare cause of the right upper quadrant pain. This is subtype c: lump kidney.
