Wernicke encephalopathy following hyperemesis gravidarum

Acute confusional state, severe vomiting and visual disturbances in pregnant patient (12 weeks)

Patient Data

Age: 25 years
Gender: Female

Bilateral symmetrical low T1, high T2 and FLAIR signal intensity with restricted diffusion of the medial vestibular nuclei, prepositus hypoglossal nuclei, periaqueductal area, tectum of the midbrain, mamillary bodies, and medial thalami.

MRI features of Wernicke encephalopathy (Wernicke-Korsakoff syndrome)

 Wernicke encephalopathy is due to vitamin B1 (thiamine) deficiency. The treatment should include high-dose parenteral thiamine.
